DESCRIPTION:As we are coming to the close of 2021\, we thought it would be great to get as many of us together as possible for the last meeting of the Long-term Care Council. \nThe meeting will be held at a later time on 12/1/2021: 8pm EST to accommodate work schedules so more can attend.   \nWe will have a brief update on current projects and future plans; Kacy will update about the ACP study group and Ken will talk about the website.
