On September 22th, the Long Term Care and Elder Council hosted a study group on Advance Care Planning: the basics and barriers. As a well-rounded group of clinicians, caregivers, advocates, and individuals new to the advance care planning (ACP) space, we discussed the complicated process of planning future medical care for oneself and for loved ones. Participants shared stories of how difficult it can be for patients to receive care that aligns with their wishes and preferences, even after they complete documents like an advance directive.
